Answering my own poll, in Q1 I have actually logged a formal complaint twice. Many times makes me sound awful though so I'm not selecting that ;-)

Hmm, I just complained to my team lead today about one of my coworkers who has sexual phone (and webcam) conversations at work on an almost daily basis. I mentioned it knowing that it would be completely informal and I wouldn't need to sign anything. If I had to go through HR and have my name tied to it I would have just kept my mouth shut.

The one incident in which I did actually file a complaint was when my ex who worked at the same company was stalking me and using company e-mail to do so (not very smart). He was fired, but there were so many other reasons to fire him anyway so it wasn't traced back to me.

Yeah, that would be serious enough.

I think it would have to be so serious, that you would not look worse then them. If it was petty, then you and not them would look like a difficult person.
